Dean College Athletics does not take risks when it comes to the health and well-being of its student athletes. To ensure that we have the necessary information required to
allow you to attend preseason practice, please complete all of the following forms on the Preseason Form Checklist:
The Eligibility form requires a signature on the second page.
Please include your cell phone number and email address.
The Medical History form requires that the insurance
information section is completed and that you attach a copy of your insurance
card (front and back) even if you have already sent a copy to the Wellness
Office. This copy is to be used by our Athletic Trainer. If you are purchasing
the Dean School Insurance, circle “yes” and write “Dean Insurance.” In this
case, you do not need to send in a copy of an insurance card. You must also
complete the Emergency Contact section with an individual other than your
The Dean College Sports Medicine Concussion Management Protocol form must be filled out before participating in a sport as it offers information on how to notice the signs and symptoms of a concussion and how to care for yourself or others with a concussion.
The Student Code of Conduct Form includes information on conduct, academic performance, transfer eligibility, hazing, and equipment/apparel. Please read the entire form and sign at the bottom.
The Notice of Privacy Practices form requires that you
read the instructions and sign at the end of the document.
It is an NJCAA requirement that each student-athlete has a current
in-force physical exam on file before he/she may participate in any practice,
preseason training, conditioning training, scrimmages, or games.
Mandatory recommendations (limited
or unlimited) for Physical Activity/Participation in Athletics needs to be
completed by your physician in order to participate.
First year students must have a valid
physical exam on file with the Health Services Office, along with your
immunizations records. Your physical must cover entire practice and playing
seasons. If it does not, then you will be required to get an updated exam
during the year.
Returning students should have received
a physical exam form in their Dean Email account to be completed by your doctor
before they may participate. If you did not receive your form via email, please
email or call Jeanny Lacey at (508) 541-1816.
