Massachusetts law requires all college students to submit
health forms inclusive of a physical examination, certificate of immunizations,
medical history, etc.
Student are to complete pages 1, 2 and 5. Your doctor or health care provider MUST complete
pages 3 and 4 (physical exam and certificate of immunization).
Completed forms must be returned to the Health Services
office at email@example.com
or faxed to Attn: Dean College Health
Services – 1-508-541-1925. All forms
must be received by the health services office PRIOR to the student’s arrival
State Law requires all full-time and part-time college students enrolled in 9
or more credits to participate in a qualifying student health insurance program
(SHP) or in a health insurance plan with comparable coverage.
Dean College offers a comprehensive Student Health Plan (SHP) that
exceeds the minimum standards required by the state. It is available to
students without health insurance, for students who would like to supplement
their own health insurance, or for those who do not participate in a SHP plan.
The health plan, offered by Consolidated Health Plans (CHP), is specifically
designed to meet student needs. It provides 24 hour a day coverage for a full
year, from August to August,
and provides coverage in the Dean College/Franklin/Milford area. The Dean
College Student Health Plan also provides coverage for services anywhere in the
United States and abroad.
insurance carriers owned and operated outside the United States or coverage
through foreign national health service programs are not in compliance with
Massachusetts State regulations and will not provide the student with
comparable health insurance coverage.
ensure compliance with Massachusetts State law, all international students are
