In order to best understand the work that you do within the greater community, and to celebrate your achievements, please take a few moments to track your community engagement experiences.
Please only enter one experience per submission. For example, if you volunteered twice (even if it is at the same location), you will complete this survey twice.
An executive board member of each student organization will fill out this form for any project/service that the group has participated in.
If you are a member of a leadership/athletic team (i.e. Orientation Leader, Volleyball Team), a representative from the team will fill out the form on the group's behalf.
