Authorization for Transfer Credit
If you are a current student who is thinking of taking courses at another institution with the intention of transferring the credits to Dean College, you must complete this form and submit it to the Office of the Registrar.
Change of Address Form
Use this form to update student and/or parent address information. Print this form out, fill in the fields with your updated information, and submit it to the Office of the Registrar.
Application for Re-Admission/Re-Entry
If you are a former student of Dean College and intend on returning to the institution after having been gone for any period of time, print and complete this form, and submit it to the Office of the Registrar.
Withdrawal/Departure Form
Students who wish to withdraw voluntarily from the College must complete this form with their Advisor, obtain the necessary signatures, and submit it to the Office of the Registrar located in Dean Hall.
