Online Waiver Form

Massachusetts State Law requires all full-time and certain part-time college students to participate in a qualifying student health insurance program (SHP) or in a health insurance plan with comparable coverage.  A part-time student is defined as a student enrolled in at least 75% of the full-time course load.  A qualifying health insurance plan is one that provides reasonable and comprehensive coverage of health services that include: preventive and primary care, emergency services, hospitalization benefits, ambulatory patient services and mental health services.  Any health plan must provide reasonably accessible health services to the student in the area where the student attends school.  Health insurance policies from other countries are not acceptable. 

Dean College offers a comprehensive Student Health Plan that exceeds the minimum standards required by the state.  It is available to students without health insurance, for students who would like to supplement their own health insurance, or for those who do not participate in a SHP plan.  The health plan, offered by Consolidated Health Plans (CHP), is specifically designed to meet student needs   It provides 24 hour/day coverage for a full year, from August to August, and provides coverage in the Dean College/Franklin/Milford area.  The Dean College Student Health Plan also provides coverage for services beyond the Dean College area.

All students are automatically enrolled in and charged for the Dean College Student Health Plan unless proof of comparable coverage is provided.  If a student has his/her own health insurance and declines enrollment in the college health plan, the student must complete the online Dean College Insurance Waiver Form.  (This link is only available during specific enrollment dates:  Fall Semester - June 1st through September 15th and Spring Semester - December 1st through February 1stIt is important to note that the burden of proof of participating in a SHP approved health insurance with comparable coverage falls on the student.  By completing and submitting the insurance waiver, the student accepts responsibility for all medical and/or mental health expenses incurred should his/her own insurance prove insufficient.

The online waiver process must be completed by the deadline indicated above and noted in the Dean college Health Form packet. Once the waiver has been submitted, the Dean College Student Health Plan charge will be removed from the student account.  If the college does not receive confirmation of the waiver process, the student’s account will be billed for the insurance and the charge becomes non-refundable. 

Students who wish to enroll in the Student Health Plan do not complete the waiver form and are automatically enrolled in and are subsequently covered by the plan.
